What Happened?

Between September 9 and September 12, 2024, an unauthorized party gained access to certain internal email accounts. Freedom Square swiftly secured its systems and initiated a forensic investigation. While the investigation is still ongoing, the review has confirmed that a limited amount of protected health information (PHI) may have been exposed during the breach.

Although there is no evidence of misuse at this time, Freedom Square is taking proactive measures by notifying affected individuals out of an abundance of caution.

Who was affected?

  • Date of birth

  • Social Security number

  • Driver’s license or state ID number

  • Passport number

  • U.S. military ID number

  • Alien registration/USCIS number

  • Financial account numbers & access details

  • Payment card details

  • Biometric information

  • Online account usernames and passwords

  • Medical record number

  • Patient account number

  • Medicare and Medicaid numbers

  • Health insurance account & group numbers

  • Diagnosis, treatment, and clinical information

  • Prescription details

  • Provider names and locations

What is Freedom Square doing?

Freedom Square has taken the following data protection and privacy measures to address the breach:

  • Engaged cybersecurity and forensic experts

  • Strengthened internal security protocols

  • Reviewed and updated data protection policies and procedures

  • Established a dedicated helpline to support those affected

What can you do?

To further protect your identity and monitor for suspicious activity, Freedom Square is offering free identity theft protection through IDX. This service includes:

2. Place a Fraud Alert or Credit Freeze

A fraud alert notifies lenders to take extra steps in verifying your identity.
A credit freeze prevents new credit accounts from being opened in your name without your consent.
